• Current insurance card/referral, if needed
• WC Information – adjustor name, Claim #, Company name and address
• Current list of medications and allergies
• Past X-rays, MRI, operative reports, bone scans or medical records
• Pair of shorts if problem involves the knee or a tank top if being seen for your shoulder
