Disability Forms Many of you may have disability insurance forms. We require five working days to fill these out as the forms often take substantial amounts of time to complete during non patient hours. There is a $40 charge for disability forms (EDD/private) if 2 pages and $60 if more than 2 pages. . This is to be paid in advance at the time the forms are dropped off. If medical records are requested,we require 7 days to produce and a signed letter requesting them. These are provided to you. Records required by attorneys and outside copy service are charged $25 minimum and $0.25 per page over 100 pages. |
